Abstract. The definitive technical investigation of the Therac-25 radiation-therapy accidents, in which at least six patients were massively overdosed between 1985 and 1987. Identifies the interaction between a software race condition triggered by fast operator editing and the removal of hardware interlocks present on earlier models.
